Since 2005, Bayhealth Home Care has partnered with Quality Improvement Organizations across the country to assist home health agencies and staff in transforming the way care is delivered. Bayhealth set a goal of improving patient care by targeting the reduction of hospital re-admissions of patients with congestive heart failure.
